Chronic disease management, including home health assessments, can identify clinical problems by assessing physical health and safety, hearing, cognitive ability, nutrition and social well being.
PCM provides a service on behalf of the client’s GP with a focus on prevention and detection. This gives GP's the option of using PCM as an external provider as needed i.e. if the number of 75+ clients is above the capacity for of inhouse practice nurses.
