GP Management Plan (GPMP)/Team Care Arrangement (TCA)

General Practitioner Management Plan (GPMP)

For patients with a chronic illness or terminal condition OR patients who have complex care needs requiring care from a multidisciplinary team (also eligible for Team Care Arrangement). A GP Management Plan can be provided independently of a Team Care Arrangement however, it is expected that in most cases a patient with complex care needs would have both services. It is mandatory to follow the preparation of a GP Management Plan before coordinating a Team Care Arrangement.

For patients to be eligible to access rebates under the allied health and dental care items, they must have both a GP Management Plan and a Team Care Arrangement in place and have previously claimed on Medicare. The GP may be assisted by a practice nurse, Aboriginal Health Worker or other health professionals and require the patient’s consent.

GP Management Plan Review
Frequent reviews ensure the continued suitability of the GPMP and management of the patient’s chronic condition. This is performed every six months unless a major change occurs in the patient’s health.

Team Care Arrangement (TCA)

Overview
For patients with chronic or terminal conditions, and who require ongoing care from a multidisciplinary team of their GP and at least two (2) other health or care providers.

A rebate can be claimed once the patient’s usual GP (or a GP in the same practice) has coordinated the development of the Team Care Arrangement. Both a GP Management Plan (GPMP) and a Team Care Arrangement (TCA) must be claimed to enable the patients’ access to allied health providers (EPC).

A GP may be assisted by a practice nurse, Aboriginal Health Worker or other health professional and must have the patient’s consent.

This service can be provided for patients who have a current GPMP.

Team Care Arrangements Review
This is for patients who have TCA in place and who will benefit from a team-based review. This is performed on a six monthly basis, unless there is a major change in the patient’s health.

A rebate can be claimed once the GP has coordinated a systematic team-based review of the patient’s progress against the TCA goals by completing the relevant requirements. The GP may be assisted by a practice nurse.
The service must include a personal attendance by the GP with the patient as part of the MBS item.
